I just took 2 mins to re-read that tutorial it you said you followed and it tells you to install cIOS as base 56 to 249: BUT THEN TELLS YOU TO INSTALL 57 over that and then 58 over that!.... try to just install 249 as base 56...
The guide looks wrong to me, asking you to install cios over cios in the same slot.
EDIT: I thought so: the guide you used is wrong and out dated.
ios 56 should be installed to 249 in d2x installer, 57 to 250 and 58 to 251.
