I've played mass effect 3 from beginning to end. Afterwards I installed the alt. ending dlc and everything worked fine.
Probably a bad copy of the game.

edit:
I used BLUS30853

The warning you get in ps3gen is normal, I've had it to.
And yes, I used 1.04 retail update, unpacked it and then repacked it with psn-package_npdrm.exe.
I'm no expert, but the error you get while running the game indicates that it's a bad disk so it has nothing to do with the update package. Otherwise you'd get a 8000xxxx error.
Try reripping or downloading the game and see if the problem is still there.

Did you download the game or did you rip the game from a Retail BluRay Disk?
If you have downloaded it, I guess you got the TB fixed Mass Effect.
First you have to do is download latest Retail Patch for Mass Effect BLUSXXXX.
Then unpack the retail patch and copy all files to your downloaded game.
Replace the new files with the old ones.
Your game is now orginal.
Then build the game with ps3gen.
Convert the retail patch which you have downloaded into a dex .
The game should work.
