download this https://www.dropbox.com/s/1mtofp7nrpk2raz/toggle_qa.pkg install it then once installed click on it. Your screen will go black and your ps3 will beep 3 times. to check it has worked got to network settings and do the button combo L1L2R1R2L3 and down on d-pad

well I had a similar experience going from rebug 3.55 to rebug 4.21
I had installed the toogle QA thingy, but it never beeped three times like it was supposed to (probably because I didn't need to dehash syscon as my PS3 was never downgraded).

What I did (and what I advise you to do) is install Rebug Toolbox app... this app will clearly indicate wether QA Toogle is enabled or not....

Turned out the QA toogle pkg never gave me 3 beeps, but the Rebug Toolbox showed it was actually activated.

Toggling Qa flag on and off, will work no matter if your console has been downgraded or not. It has many other uses than just dehashing the syscon checks.

If the app doesn't beep 3 times and reboot, then either reinstall like I said. Or you don't have your blu ray drive attached.
