I heard about this problem happening only on PS3s with Rogero 4.30 CFW that have never been online before. Even if you don't wanna play online games just Sign In to PSN then Sign Out, Then try to use ReactPSN. I have used it with Skyrim's Dragonborn DLC and it worked 100% fine.
Oh and the USB device that has your exdata folder on it must be is in the USB port on the right closest to the blu-ray drive, And it must show as 'dev_usb000' in Multiman.

EDIT: I got this from 'reActPSN Beginners guide for 4.xx CFW' at ps3repack.
Troubleshooting (Only refer to this if you tried the guide and were unable to activate content)

I followed all the steps but I am unable to activate any content.
I get the following message when I apply a theme installed from the guide
"This theme cannot be used.
You maybe able to use it after you signing in.
sign in with the account that was used to purchase this theme and Select it again."

Solution:
This problem seems to occur on Rogero's CFW on ps3's which have never been online and do not have a PSN act.dat.
Either switch to rebug or you may need to get a PSN act.dat

If you have not created a PSN account already, do so.

After creating your PSN account , activate your ps3 for this account
On the PS3 XMB , go to the Playstation Network Tab ->Account Management->System Activation->PS3 System

Once your account is activated , go to the PSN Store and download some free content which requires activation.
You do not require to have the game to download the content.
Just before you start to download the content , it will say Activating and then start downloading.If not , try some other content.
This is required to create an act.dat file for your PSN account.
You could try
- Back to the future part 1 trial (100kb).
- GT5 Scion FR-S '12 (game vehicle)
- GT5 Corvette C7 Test Prototype '12 (as above)
- Max Payne 3 Gorilla Warfare Pack (game add-on)
- Max Payne 3 Disorganized Crime Pack (game level)

Now confirm that you have the act.dat file for your PSN account.
Sign out of PSN and Open mM’s File Manager .
Browse to hdd0/home/0000000X/exdata (The x indicates the profile number on the system)
You should find an act.dat file in this directory.
Open it by pressing X on it.It should have random numbers and alphabet in hexadecimal form.
This means that you now have the required PSN act.dat file.Back it up on your PC as well.

After this , you are no longer required to be online and can log off from PSN/disable network connection if you wish.

I highly suspect this is how so many people are being identified and having their accounts banned. I mean, Im sure Sony can see what different account names we have and when all of us have the same one its kind of obvious.

Not likely, I could be mistaken but as far as I know, the root of your ps3 defines the account as numbers 001, 002. The name you decide to put in your XMB is completely unknown to Sony.

Nothing is completly unknown.

Don´t mix things up here. You have your local account and your psn account.
To trigger your act.dat you don´t need to login as aa user.

trigger act.dat

-Log in with any local user
-Log in to PSNstore with your PSN account (If you don´t have one, create one)
-download any free content
-act.dat will be downloaded to exdata folder of the current user

This has to be done only once!

Activating content with reactpsn (rap files)

- put exdata folder with raps to the root of your usb
- plugin to the right port (near the BD drive)
- create a user named aa and login
- run react by pressing -> automically restart -> ignore the warning with
- aa has been renamed to reactpsn with a count of created *.rifs and *.edats
- *.rif files have been created in the exdata folder
- Content is activated

To activate new stuff rename the react user to aa and do above again.
It doesn´t matter if the pkg is installed before or after this.
You can create thousands of *.rif files at once if you want to.


Activating c00 demos (like castle crashers or outland)

- install pkg
- log in as any local user
- run reactpsn with and hold till you are back in xmb
- demos are now full games

After the first start, the xmb icon will change.

What are c00 games? is Tetris one by any chance? still havent been able to successfully install that game. lol.

@sinsizer I was under the impression ReactPSN generated a generic ACT.DAT, and that PSN therefore was not needed ever at all?

.. also I found this quote from the ReactPSN 2.20 thread at tortuga-cove:

And for some who don't have an act.dat, and reactPSN does not create one. There are universal ones out there if you google it.
Is this true? or should 2.21+higher generate one?

if you dont have the act.dat file (located in dev_hdd0/home/0000000X/exdata on your ps3) you have to use reactPSN once to get a universal one, there are plenty of guides on this. Your "act.dat" file go in "c:/cygwin/home/YourUsername/.ps3/exdata"

leghari80 succeed on this switching from rogero 4.31 v2.05 to rebug 4.30.1, and on rebug it works without needing to ever connect the console to internet, I will try to talk to rogero on this
EDIT 4:

Out of interest can anyone with REBUG 4.XX cfw (clean install: no content on your PS3) generate a ACT.DAT with ReactPSN and upload it for testing on rogero 2.05?

@Dnb I couldnt find act.dat on my Ps3 and React didnt create one, If your still struggling to get your React to work try @snkysnake02 act.dat at post #2, it worked for me

Downloading free stuff to get the act.dat is the easiest way, as far as I know.
It should be possible to create one locally, but every act.dat is build with your idps for your account.
For example: If you have already purchased something in the store before, with your account, the act.dat contains entries for this games if you download it.
This needs some further research.

@westnz

c00 is a folder in the game.pkg. All these games with c00 folder can be activated with circle. No need for a rap file.

My CFW machine is strictly offline for PSN, never been on, never signed in, never bought anything.

React should support this state of play, but TBH it looks as if its Rogero 2.05 that is the issue here:

if: rogero 2.05 + been on PSN = reactpsn works
if: rogero 2.05 + never been on PSN = reactpsn fails (although some people still swear black is blue that it works for them)
if: rebug 4.(any) + been on PSN = reactpsn works
if: rebug 4.(any) + never been on PSN = reactpsn works

.. I may have to switch to rebug...
