I had same problem using Rebug 4.21 and resigning that eboot with trueancestor. So i replace with original eboot, use multiman 4.11.13 (it resign some eboots to use cfw 4.21) and.... PROBLEM SOLVED (PS3 Controller malfunction solved).

TA resigner 1.41 can fix this game for 4.21.1 REX.
I will add an alt option for 4.21.1 later.
3.55? No hope yet, cuz some interface has been changed since 4.00 fw.
