"Does Sony put the games that have exploits in them back on the PS store after fixing the exploits, or are they removed permanently?"

Of course they are put back up. The game itself is never patched, the firmware is patched. After the firmware is patched, the game will be back up on the store.

Nope. If the name of the game is announced, Sony will take it down as soon as they are able. That and Sony themselves have patched games without even the public ever knowing the game had a hole. Don't assume they themselves can't look for an exploit in an announced game.
