In short this thread has lead nowhere here is the detailed summary
After reading various thread and posts I have come to a conclusion how users are getting banned:-
1.Early play i.e playing the game before release.
2.Using homebrew online.(only dumb people do this)
3.Playing Black Ops2(this game is injected with a backdoor trojan/malware which reports your hdd structure to Sony)
4.Using Cracked Online pass for playing multiplayer(seriously!!!)
5.New game updates also may contains a backdoor trojan/malware like black ops2.
6.Playing psn games not downloaded on your account with modified edats
7.Using cheats or modified eboots to play online.(most uncool that has lead to ban waves)
8.Loading multiman when online be advised even if multiman says it is in stealth if you load it while you are connected online it can be detected as the payloads used in this homebrew is not found any where else apart from CFW users.

In short stay the **** out of psn
